You can get treatment inside a clinic and stay there until you get well, or get treatment at home. You can join support groups and talk to other people that confront the same problems as you. This will help you a lot, as you will see that other people can beat addiction and you will realize that so can you. Last but not least, you will talk to a rehab doctor who will analyze your case and indicate exactly what you need to do in order to start a new life.
